Qualifications
- Applicants must be able to meet the criteria for a Designated Coordinator as assigned by 245D which includes a combination of work experience and education.
- Diploma in a human service related field from an accredited post-secondary institute, 3 years of full time (or equivalent) work experience providing direct care services to persons with disabilities or persons age 65 years and older OR
- A minimum of 4 years experience providing direct care services to people with disabilities under the supervision of someone who meets the above criteria
- Must be at least 18 years of age
- Ability to pass a Department of Human Services background check
- Must have a valid driver’s license
- Mankato, MN: Reliably commute or planning to relocate before starting work (Required)
- Experience in a Lead or Home Manager/Designated Coordinator role preferred.

Job description
Designated Coordinators oversee the day to day functions of a residential program.
Some responsibilities include:
- Partnering with the individuals we support to ensure they have quality services that help them live their best life and that services are provided in accordance with their community support plans and MN Statute 245D.
- Oversee day to day functions of the program you are supervising including maintaining a program budget and identification of program needs.
- Provide oversight and guidance to the Direct Support Professional role including scheduling, performance management, training, and serving as a role model

Direct Support Professional - OPEN
Direct Support Professional

Qualifications
- You have the dedication to provide culturally competent services to uphold each individual’s needs, preferences, and interests
- Reliably commute to worksite or planning to relocate before starting work (Required). Primary locations include: Mankato/North Mankato/St. Peter/Kasota, MN
- Be able to transport clients in a company owned vehicle for community outings and appointments as deemed necessary
- Either have a valid driver’s license, AND active car insurance, a reliable vehicle and/or have reliable transportation to work

Responsibilities
- Employees should expect to work minimally with 1 individual at a specific location
- Provides direct care to those with intellectual and developmental disabilities.
- Assist individuals with activities of daily living care for themself including: bathing, dressing, toileting, transferring (getting in and out of bed or chair), eating, and continence AS NECESSARY
- Assist individuals with Instrumental Activities of Daily living including: Meal planning/preparation, shopping, using the telephone, housekeeping, money management, and taking medications, AS NECESSARY
- Assist in planning meaningful activities in the community or at home based on the individuals preferences, interests, and passions
- DSP will work with someone to enable them to do things independently\

Nurse
Medical Assessment and Care
- Conduct regular medical assessments of clients, including monitoring vital signs and medical conditions.
- Administer medications as prescribed by a licensed medical professional and maintain accurate records.
Health Care Planning
- Develop, implement, and update individualized health care plans for clients in collaboration with the designated coordinator and other stakeholders.
- Ensure that health care plans address each client’s specific medical needs and goals.
Emergency Medical Response
- Be prepared to respond to medical emergencies promptly and appropriately.
- Conduct regular drills and training for staff on emergency medical response procedures.
Medication Management
- Ensure that medication administration is accurate, timely, and follows proper protocols.
- Monitor clients for any medication-related side effects or adverse reactions.
Client Education
- Educate clients, staff, and caregivers on medical conditions, medications, and any necessary medical procedures.
- Promote health and wellness among clients.
Health Records and Documentation
- Maintain accurate and up-to-date health records for clients.
- Document all medical care, assessments, and medication administration.
Regulatory Compliance
- Stay informed about all relevant healthcare regulations and licensing requirements.
- Ensure that the facility complies with all medical regulations and standards.
Collaboration with Healthcare Providers
- Work in collaboration with physicians, specialists, and other healthcare providers to coordinate medical care for clients.
- Communicate with outside healthcare professionals to ensure continuity of care.
Infection Control
- Implement and oversee infection control measures to protect clients and staff.
- Provide training to staff on infection prevention and control procedures.
Quality Assurance
- Participate in quality assurance programs to assess and improve the quality of medical care provided to clients.
- Regularly assess and report on the quality of care.
Client Advocacy
- Act as an advocate for clients, ensuring that their medical rights and best interests are upheld.
- Address any client concerns or grievances related to healthcare.
Training and Development
- Identify training needs for staff related to medical care and healthcare procedures.
- Promote professional development among staff in healthcare-related areas.
Medication Inventory and Management
- Manage and maintain an inventory of medications and medical supplies, ensuring they are safely stored and properly labeled.
- Monitor and reorder supplies as needed.
Policy and Procedure Updates
- Ensure that all medical policies and procedures are up to date and in compliance with regulations.
- Communicate changes and updates to staff and provide necessary training.
Supervision of Medical-Related Financial Matters
- Oversee and manage medical-related financial matters, such as insurance claims and medical billing.
- Maintain accurate records of medical expenses.
Client Privacy and Confidentiality
- Uphold strict client confidentiality standards and ensure that staff members do the same.
- Protect and secure medical records and personal health information.
Unacceptable Behaviors Enforcement
- Enforce the organization’s policies regarding unacceptable behaviors related to medical care, including medication management and infection control.
- Address and report any violations of these policies.
