Our Approach
Resolute Care values partnerships including with the county case manager, the individual’s family, and other supports to best meet the person’s needs. We aim to be flexible and innovative, specifically when exploring new services and supports to offer an individualized program. Staff incorporate a person-centered, and strength-based approach when assessing the individuals needs.

Resolute Care, LLC is a Minnesota-based provider of high-quality Home and Community Based Services (HCBS) for individuals with disabilities and those aged 18 and above. Resolute Care believes that everyone should have the chance to live their best life, regardless of any obstacles they may face, and we are committed to helping them achieve that.
Our Community Residential Settings are licensed under 245D MN Statutes and are designed to feel like home, located in safe residential neighborhoods in small towns and rural areas. We offer a wide range of services for adults aged 18 and above with various developmental and intellectual disabilities, as well as mental illnesses, substance use disorders and co-occurring disorders.

Community Access for Disability Inclusion (CADI)
The CADI waiver helps eligible Minnesotans with disabilities receive home and community based support as an alternative to nursing facility care. Services are chosen around assessed needs, personal preferences, and an approved support plan. For adults exploring care with Resolute Care, the conversation starts with daily routines, meaningful goals, and individual support needs.
CADI is a funding program, while the individual's support plan identifies the help they receive. A planning conversation may cover personal care, household routines, access to the community, and the support needed to live safely. Resolute Care works with the individual and their support team to discuss how those priorities relate to our services.

Supportive Living Services (SLS) at Home
Supportive living services focus on the practical parts of everyday life at home. Based on the individual's plan, support may include personal care, daily routines, household tasks, and opportunities to build skills and take part in the community. Resolute Care's person-centered approach respects each person's choices while encouraging confidence and independence.
Daily living support can involve reminders, encouragement, and hands-on assistance, depending on assessed needs. One person's priorities may center on personal routines and household skills; another may need more help joining activities or making everyday choices. Planning considers the person's abilities and preferences, with support aimed at participation and skill development.
At Resolute Care, these conversations include the individual and their support team. We look at what matters to the person, where assistance is needed, and how the approved plan can support a meaningful life at home and in the community.
Developmental Disabilities (DD)
The DD waiver helps eligible people with developmental disabilities or related conditions access services in community settings. Support planning considers both everyday assistance and opportunities to develop or maintain skills. We work with individuals, families, and support teams to understand what a meaningful, connected daily life looks like for each person.
Goals might include communicating choices, learning household skills, maintaining familiar routines, or taking part in community activities. The emphasis is on the person's strengths as well as areas where support is needed. Family members and other people the individual chooses can contribute to planning, while the individual's own preferences remain central.
The DD waiver has its own eligibility and service requirements. Your county or tribal agency can explain the assessment process; Resolute Care can discuss our program and the individual's proposed supports.
Brain Injury (BI)
The Brain Injury waiver can fund home and community based services for eligible people with an acquired or traumatic brain injury. Support is tailored to assessed needs and an approved plan. Daily routines, assistance with everyday activities, and community connections can be important parts of the conversation about services at Resolute Care.
When considering ongoing support after a brain injury, it can help to describe which parts of a typical day require assistance and which activities the person wants to keep doing. Conversations about routines, communication preferences, personal care, and community participation help the support team understand those priorities.
Resolute Care focuses on person-centered daily support. Eligibility for the BI waiver and the specific services that can be funded are determined through the county or tribal agency's assessment and authorization process.
